Abstract

결론
저자는 1996년 11월 원광대학교 치과대학 구강악안면외과에서 하악골에 발생한 범랑아세
포종으로 인하여 하악골 부분 절제술을 시행받고, PMCB와 동종 하악골 crib을 이용하여 연
속성이 결여된 하악골을 재건한 환자를 1997년 11월까지 1년 동안 주기적인 검진을 시행하
였다. 동종골의 안정성과 골형성능을 평가하고자 환자의 임상기록과 방사선 사진을 이용하
여 본 연구를 시행하여 다음과 같은 결과를 얻을 수 있었다.
1. 창상 이개, 술후 감염, 이식골의 비유합등과 같은 중대한 합병증을 보이지 않았다.
2. PMCB를 이식하기 위해 사용된 crib으로는 동결-건조(Freeze-drying) 동종 하악골
(allogeneic mandibular bone)로 매우 양호한 심미성을 보였다.
3. 재건한 하악골의 길이는 58mm, 높이는 30mm이었으며, 술후 약 1년 뒤의 골 흡수율은
약 22%이었다. 골 흡수율은 다소 높았으나 골의 입자 형태에서 점차적으로 균질한 형태와
밀도를 갖는 골로 경화됨을 알 수 있었다.

To repair bony defects with transplanted bone in the body, fresh autogenous bone is
undoubtly. A retrospective study was made of a consecutive patient who underwent
mandibular reconstruction with PMCB and allogeneic mandibular crib in November 1996.
Free autogenous iliac bone in the form of particulate cancellous bone and marrow was
densely packed into the allogenous mandibular crib that was adapted to bridge the
mandibular discontinuity defect. Freeze dried allogenous mandibular bone was traded by
Wonkwang bone bank. Freeze dried allogenous mandibular bone was used as allogeneic
crib carrying the PMCB. This case was undergone successful healing with the formation
of a continuous bony union with the remaining mandible. The rate of resorption was
assessed by sequeal panoramic radiographs, that is, a day of postoperative period, 3
months of postoperative period, 6 months of postoperative period, 12 months of
postoperative period. The mean horizontal dimension of the mandibular defect was 58mm
and the mean vertical dimension of the reconstructed segments was 30mm. The bony
height reconstructed segments retained about 78% of the bony height of a 1-year period.
We estimated that autogenous PMCB grafts and allogenous crib grafts were the most
successful and esthetic grafts in mandibular discontinuity reconstruction.
